Liability Waiver Agreement

For: Bridget Center of Wisconsin, Inc.

Event/Activity: Visiting Bridget Center

Location: 9003 Townline Rd. Kewaskum, WI 53040

I acknowledge that I am voluntarily participating in the above-mentioned event/activity hosted by Bridget Center of Wisconsin (hereinafter referred to as "BC").

I understand and acknowledge the following:

  • I am aware of the inherent risks associated with participating in this event/activity, including, but not limited to, risk of injury from: falls, weather conditions, other participants, exposure to elements, property damage, use of tools and machinery, etc.

  • I am participating in this event/activity at my own free will and with full knowledge of the potential risks involved.

  • I am responsible for my own safety and well-being during this event/activity.

  • I will follow all instructions and guidelines provided by BC and its representatives.

In consideration of being permitted to participate in this event/activity, I hereby:

  • Release and Discharge:

    Release, discharge, and covenant not to sue BC, its directors, officers, employees, volunteers, and agents (hereinafter collectively referred to as "Released Parties") from any and all claims, demands, liabilities, actions, and causes of action of any kind whatsoever, whether known or unknown, arising out of or in any way connected with my participation in this event/activity, including but not limited to any injury, loss, or damage to my person or property, or any death resulting therefrom.

  • Indemnify and Hold Harmless:

    Indemnify, defend, and hold harmless the Released Parties from any and all claims, demands, liabilities, actions, and causes of action of any kind whatsoever, whether known or unknown, arising out of or in any way connected with my participation in this event/activity, including but not limited to any injury, loss, or damage to my person or property, or any death resulting therefrom.

  • Assume all Risks:

    Assume all risks of any kind whatsoever, whether known or unknown, arising out of or in any way connected with my participation in this event/activity, including but not limited to any injury, loss, or damage to my person or property, or any death resulting therefrom.
